How to Access

To register/apply for respite at the Wimmera Respite Service, please contact the WRS Co-ordinator on 03 5398 2166, or download the following form;

Respite Information Sheet
To be completed by all clients of service. Please complete all information, and contact an agency representative should you have any questions.

On receipt of the Respite Information Sheet, the WRS Co-ordinator will assist you to complete a Client Assessment report, to ensure we meet your needs.

Agencies wishing to refer a client
Must contact the WRS Co-ordinator to book clients into the service.
